ResponsibilitiesThe RN Navigator functions on the multidisciplinary team as a patient advocate, educator and counselor. Responsible for ensuring assigned patient population receive quality and comprehensive services. Coordinates patient care throughout the continuum in collaboration with the multidisciplinary team and serves as a clinical resource with expertise in the area of specialty. Serves as a liaison throughout the hospital system and in the community regarding services provided for their unique patient population. Incumbent will provide expert nursing care, which includes: direct clinical practice, consultation, education and research. Demonstrates ability to act as a mentor.
QualificationsEducation• Graduation from a school of Nursing or successful completion of sufficient units in nursing qualifying for state licensure required Experience• 3 years of recent nursing experience in the area specialty required Licenses and Certifications• RN - Current State of California Registered Nurse license required• BLS - Current Basic Life Support (BLS) for Healthcare Providers by American Heart Association (AHA) required• NATIONAL - National Certification related to specialty and applicable to role OR Bachelor of Science in Nursing (BSN) Degree required
