Qualifications

• Current LPN or RN licensure from the Board of Registration in Nursing.
• 1-3 years of nursing experience, particularly in a hospital or healthcare setting.
• Minimum 2 years of experience in a Skilled Nursing Facility (SNF) or sub-acute care environment.
• Desirable background in administrative or admissions roles within healthcare.
• Strong interpersonal, customer service, and communication skills essential for patient interactions and collaboration with facilities.
• Proficient in computer applications including electronic health records and databases.
• Vaccination against seasonal influenza and COVID-19 required, with an option for medical or personal exemptions.

Responsibilities

• Communicate with Skilled Nursing Facilities (SNFs) for patient bed offers for Southcoast Hospitals.
• Coordinate bed offers from SNFs in conjunction with case managers at SLH, CMH, and Tobey.
• Oversee the referral process for bed offers to ensure smooth transitions for patients from acute care to SNFs.
• Manage patient needs and issues related to the discharge process, including scheduling and insurance concerns.
• Build and maintain strong relationships with SNF accounts to maximize bed offers and expedite patient movement.
• Document bed offers, bed flows, and discharge calendars to promote awareness of care access.
