Job Title: Registered Nurse (RN)

Shift: Friday, Saturday, Sunday plus 1 weekday - no call requirements
Hours per Shift: 4x10 or 3x8 plus 8, 40 hour work week

Experience: 2 years of professional nursing experience in public health, such as an acute care or home care setting or hospice inpatient unit
License: Currently licensed as a Registered Nurse in the state of South Carolina or holds a current compact/multi-state license in a recognized compact state
Certifications: Current CPR certification

Must-Have: Valid South Carolina driver's license with a good driving record for home visits

Description:
To provide nursing care for patients following established standards and practices in accordance with the Nurse Practice Act.
To apply professional nursing theory and practice to assess, plan, implement, and evaluate patient care.
The Registered Nurse (RN) plans, organizes, and directs hospice care.
The RN supervises and coordinates care to patients with terminal illnesses and compromised health states.
The RN provides comprehensive skilled nursing services to qualified patients in their homes or hospice inpatient settings.
This enables the stated goals and objectives of the agency to be achieved.
The nurse works with minimal daily supervision while performing interdependent practice in the field.
The RN is responsible for supervising hospice aides, administering medications, managing symptoms, providing education, and delivering end-of-life care.

Job Duties and Responsibilities:
Works in combination with the family/caregiver and interdisciplinary team to meet the medical and emotional needs of the patient.
Provides continuity of care through direct hands-on care that follows the patient's plan of care and current standards of practice for nursing.
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition.
Obtains and receives physician orders as required.
Provides and maintains a safe environment for the patient.
Teaches self-care techniques and ensures arrangements for any necessary services and items.
Assesses patient condition, administers medications, and evaluates/treats symptoms and end-of-life needs.
Completes and maintains accurate clinical notes/documentation regarding the patient's condition and care provided.

Actively participates in quality assessment performance improvement teams and activities.
Assumes responsibility for personal growth and development.
Maintains and upgrades professional knowledge and practice skills through continuing education and in-service classes.
Supervises care provided by hospice aides to ensure treatment goals are met.
Delegates responsibilities when required.
Coordinates with other disciplines, agencies, families, etc., to ensure that patient care needs are met.
Participates in on-call duties as defined by policy.
Performs other job duties and responsibilities as required.

Frequent standing, walking, bending, crouching, stooping, and driving.
Frequent lifting or moving patients.
Intermittent lifting, moving, or carrying up to 50 lbs.
Frequent pushing or pulling over 50 lbs.
Frequent use of hand dexterity and eye/hand coordination.
Constant reaching with hands/arms.
Frequent talking or hearing.
Corrected hearing and vision to normal range.
Exposure to blood, body fluids, or tissue.
Frequent exposure to communicable diseases and unclean environments.
Requires occasional on-call evenings and weekends based on department needs.
