Full Job Description
Location: Suffolk, NY

Schedule: Full-Time, Exempt

Salary Range: $115,000.00 annually

Your Role

You will oversee the clinical care of an assigned caseload, ensure optimal outcomes, compliance with regulatory standards, and strong partnership with clients, families and healthcare professionals. You will assess client needs, develop personalized care plans, and support caregivers in the field with expert guidance and clinical leadership.

What You'll Do
  • Conduct client assessments, develop care plans and adjust as needs evolve
  • Provide clinical supervision and direction to caregivers and nurses
  • Collaborate with families to set goals and align daily care with those priorities
  • Maintain accurate, timely and compliant client documentation in the EMO and home records, ensure prompt communication of status changes and oversee information sharing with families, practitioners and stakeholders to support continuity of care and regulatory compliance
  • Educate clients, families and staff to ensure clear understanding of care needs
  • Identify caregiver training opportunities and provide coaching for growth
  • Support hiring, training and supervision of nursing staff
  • Participate in on-call rotation to provide consistent 24/7 client support

What you Bring
  • Graduate of an accredited nursing program, with an active, unrestricted nursing license; BSN preferred
  • Current CPR certification: CPR instructor certification preferred
  • At least 2 years of nursing experience, with one or more in home health, rehabilitation or case management strongly preferred
  • Strong skills in clinical assessment, critical thinking and problem-solving
  • Exceptional communication skills with the ability to partner effectively across clients, families, caregivers and medical professionals
  • A compassionate, professional presence with the ability to thrive in fast paced environments
