Responsibilities:- Conduct initial client assessments and continuously refine care plans.
- Provide hands-on nursing care and patient education.
- Maintain open lines of communication with patients, families, and the healthcare team.
- Manage patient care coordination for your caseload.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
Qualifications:- Licensed as a Registered Nurse / RN in the state you work in.
- Proficiency with Electronic Medical Records (EMR) software.
- Excellent planning, organizing, and communication skills.
- Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
- Valid Driver's License and reliable vehicle with current insurance
