Qualifications

• 1+ years of recent case management or discharge planning experience preferred
• Active State or Compact RN License
• BLS certification from AHA
• Strong assessment skills
• Experience in utilization review

Responsibilities

• Coordinate patient care plans and services across the continuum of care
• Collaborate with providers, social workers, and external agencies
• Ensure timely and efficient discharge planning
• Support utilization management efforts
• Ensure compliance with payer guidelines
