As an IKC Registered Nurse, RN Case Manager, you assist some of the most complex patient population in navigating the health care system and making it work for them. You are able to support and look at a patient holistically, help them navigate the health care system. You are their advocate and get 1:1 time to assist in impacting the patient experience.
Position Details - Schedule: Monday-Friday, full-time schedule with the ability to accommodate patient availability-flexibility is key. No nights, No weekends, No on call.

The Registered Nurse, RN Case Manager, functions autonomously and in collaboration with all members of the healthcare team to coordinate and facilitate quality, cost-effective care while minimizing fragmentation of the healthcare delivery system for Chronic Kidney Disease (CKD) patients or ESKD (End-Stage Kidney Care) patients. This Registered Nurse, RN Case Manager
provides coordination of care between the patient, family, physician, provider and care teams, the community, and Regional Operations Manager. Qualifications for Registered Nurse, RN Case Manager: - Degree from accredited school of nursing required; B.S.N preferred
- Current Registered Nurse (RN) license in the state of practice required
- Continuing education credits maintained as required by state of practice required
- Minimum of five (5) years' of RN experience in clinical nursing Required
- Previous Case Management experience required
- Demonstrated knowledge and understanding of data and managing to clinical, financial, and patient satisfaction outcomes
- Experience and passion for educating patients
- Current CPR certification required
- Ability to adapt to new/changing technology
- Current driver's license in state employed with positive driving record and able to meet requirements of insurance coverage required
- Basic computer skills and proficiency in MS Word and Outlook required

The Salary Range for the role is $68,400.00 - $100,400.00 per year.
