Qualifications

• Current and valid state RN license.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Basic Life Support (BLS) certification, including hands-on CPR training.
• Valid driver's license and auto insurance required.
• Associate's or Bachelor's degree in Nursing, with home health experience preferred.

Responsibilities

• Completes an initial assessment of the patient and family to determine home care needs.
• Initiates and revises the plan of care as patient needs change.
• Regularly re-evaluates patients' needs for ongoing care.
• Uses health data to determine nursing diagnosis.
• Develops a patient-centered care plan with therapeutic and rehabilitative actions.
• Administers medications and treatments as per the physician's orders.
• Counsels patients and families about care plans and health needs.
• Identifies discharge planning needs and implements them before patient discharge.
