RN Case Manager, Hospice

Full-Time | Monday-Friday | On-Call Rotation (5 Days/Month)
Territory: Northeast Twin Cities Metro (5-6 Locations)

Position Overview

We are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10-15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life.

This position offers a consistent Monday-Friday schedule, manageable caseload, and a localized territory in the northwest metro, allowing you to build meaningful relationships with patients and families.

What You'll Do

Patient Care & Case Management:
  • Conduct initial and ongoing patient assessments
  • Develop and manage individualized hospice care plans
  • Administer medications and provide palliative interventions
  • Educate patients and families on symptom management and care needs
  • Provide emotional support to patients and families


Care Coordination:
  • Manage and coordinate care for assigned patients
  • Collaborate with physicians and interdisciplinary team members
  • Participate in care conferences and family meetings
  • Advocate for patient needs and ensure access to resources


Clinical & Operational Excellence:
  • Maintain accurate and timely clinical documentation
  • Supervise and support hospice aides
  • Ensure necessary equipment and supplies are in place
  • Participate in on-call rotation (5 days/month)
  • Contribute to quality improvement initiatives
