Position OverviewWe are seeking a dedicated RN Case Manager to join our hospice team. In this role, you will manage a caseload of 10-15 patients, coordinating care across an interdisciplinary team to ensure comfort, dignity, and quality of life.
This position offers a consistent Monday-Friday schedule, manageable caseload, and a localized territory in the northwest metro, allowing you to build meaningful relationships with patients and families.
What You'll DoPatient Care & Case Management:- Conduct initial and ongoing patient assessments
- Develop and manage individualized hospice care plans
- Administer medications and provide palliative interventions
- Educate patients and families on symptom management and care needs
- Provide emotional support to patients and families
Care Coordination:- Manage and coordinate care for assigned patients
- Collaborate with physicians and interdisciplinary team members
- Participate in care conferences and family meetings
- Advocate for patient needs and ensure access to resources
Clinical & Operational Excellence:- Maintain accurate and timely clinical documentation
- Supervise and support hospice aides
- Ensure necessary equipment and supplies are in place
- Participate in on-call rotation (5 days/month)
- Contribute to quality improvement initiatives
Qualifications- Active RN license in Minnesota
- Graduate of an accredited School of Nursing
- CPR certification
- 1+ year of acute care experience required
- Hospice or home care experience preferred
- Strong assessment and clinical decision-making skills
- Valid driver's license and insured vehicle
