Full Job Description
JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)
  • This is a regional leadership position within our agency, requiring travel as needed to all counties throughout Southwest & Mid-Central Ohio where clients and caregivers reside.
    • This role works closely with and reports directly to our SW OH Regional Program Administrator (local) and/or Director of Clinical Administration & Operations (corporate).
    • For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.
  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
    • Requires back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc.
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in SW Ohio and our current + prospective employees in these communities.

SERVICE AREA: Southwest + Mid-Central Ohio
  • Primary / Centralized service areas & required travel:
    • Dayton-Middletown-Springfield Metro
    • Washington Court House-Wilmington Metro
  • Secondary service areas & required travel (aka: less frequent but occasional) span all Southwest Ohio and South-Central Ohio communities, including those in and near:
    • Dayton, Middletown, Springfield, Piqua, Urbana
    • Cincinnati & Cincy Metro areas
    • Washington Court House, Salem Township, Wilmington, Circleville
    • Columbus & immediate Columbus Metro area
  • Qualified Applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.
  • NO overnight travel required.

EMPLOYMENT & SCHEDULE TYPE: Long-term employment for Part-Time and Full-Time RNCM/Recert Nurses*
  • This role is available in two capacities, both requiring weekday availability (Monday - Friday) between 8am - 5pm, with occasional/rare evening availability (scheduled in advance), and both requiring commitment to set # of hours and consistent schedule availability.
    • Full-time, Salary (paid 2x/month via direct deposit): Requires travel throughout entire stated region (up to approx. 2-hour daily radius).
    • Part-time, Hourly (paid weekly via direct deposit): Requires travel throughout pre-determined sub-region (minimum 1-hour travel radius); Willingness to travel up to approx. 2-hour daily radius) highly desirable.
  • No On-Call Shifts / No Weekends required.
  • Weekend services are optional when available but are not required.

To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health RN Case Manager (RNCM) / Recertification Nurse to join our agency and serve existing and returning clients throughout Southwest & Mid-Central Ohio in a full-time or part-time capacity.

Compensation:
  • Full-time Annual Salary (+ PTO + Mileage) starts between $78,000 - $83,200 per year, based on minimum 2-hour travel radius and pre-determined minimum average of visits/services/recertifications per week required, plus quarterly bonus opportunities available.
  • Part-time Hourly Wage (+ Mileage) starts between $35 - $40 per hour, based on minimum 1-hour travel radius (required) and up to 2-hour / total area travel coverage (highly desired).


Job Summary As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.
  • With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:
  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Qualifications

  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
