Qualifications

• Graduated from an accredited nursing school and holds a current RN license in the applicable state.
• At least one year of RN experience in a clinical setting.
• Valid CPR certification must be maintained while employed in the clinical role.
• Must have a licensed automobile that meets insurance standards and is operational for travel.
• Possess the ability to travel to various business locations.

Responsibilities

• Manage patient care and coordinate treatment plans in a home health setting.
• Assess patients' health status and interpret medical information.
• Provide skilled nursing care according to care plans and patients' needs.
• Educate patients and families on health management and available resources.
• Document and maintain accurate records of patient care and progress.
• Work collaboratively with a multidisciplinary team to facilitate optimal care.
