Compensation: $95,000 - $130,000 annually plus

What You'll Do- Manage a home health caseload throughout Maple Grove, Dayton, Elk River, Rogers, Osseo, Otsego, Albertville, Hanover, and St. Michael.
- Complete OASIS assessments and individualized plans of care.
- Deliver skilled nursing visits and patient education.
- Coordinate care with physicians and interdisciplinary team members.
- Help patients remain healthy, independent, and safely at home.
- Maintain timely and accurate clinical documentation.
What We're Looking For- Active Minnesota Registered Nurse (RN) license.
- Strong clinical judgment and ability to work independently.
- Home health experience preferred, but not required.
- Excellent communication and organizational skills.
- Reliable transportation for local travel.
