Qualifications

• State Registered Nurse (RN) license required
• Experience in nursing case management preferred

Responsibilities

• Advocate for patients and their families throughout the care process
• Educate patients about their health status and available treatment options
• Assist patients in developing and following their personal treatment plans
• Coordinate with healthcare teams to ensure comprehensive patient care
• Review and monitor patient progress and adjust care plans as necessary
