What You'll Do- Float between home health territories throughout the Twin Cities based on patient needs.
- Complete skilled nursing visits, OASIS assessments, and individualized care plans.
- Coordinate care with physicians and interdisciplinary team members.
- Support local teams during census growth, vacations, and coverage needs.
- Deliver compassionate, one-on-one care that helps patients remain safely at home.
- Maintain timely and accurate clinical documentation.
What We're Looking For- Active Minnesota Registered Nurse (RN) license.
- Strong clinical judgment and adaptability.
- Comfortable working independently in a field-based environment.
- Home health experience preferred, but not required.
- Reliable transportation and willingness to travel throughout the metro.
