RN Case Manager Position Details:STATUS: Full-Time (mon-friday 8am-5pm w/ one 24 hour oncall shift per month)
COMPENSATION: $40-$45 /hour
LOCATION: East Valley, AZ (Mesa / Apache Junction / Gilbert / Chandler)
Your Role as RN Case Manager:As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. Your role will include:
- Conducting comprehensive physical, social, and psychological assessments.
- Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
- Managing care delivery and maintaining meticulous patient records.
- Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.
Qualifications We're Looking For:- Active Registered Nurse (RN) license in Arizona (or state of practice).
- Previous Home Health or Hospice experience (preferred).
- Effective verbal, written, and time management skills.
- Valid driver's license, reliable transportation, and proof of current auto insurance.
- Ability to perform patient transfers with a full range of movement.
- Current CPR certification and negative TB test (we can assist).
- Valid AZ Fingerprint Clearance Card (we can help obtain it).
