Full Job Description
POSITION SUMMARY

The Health and Wellness Specialist assists all Health and Wellness programs in assigned communities and is responsible for supporting communities to reach or exceed Savant standards of care and all applicable regulations. The HWS provides clinical consultation on complex care plans or conditions, while maintaining accountability in the community for resident safety and wellbeing. The HWS utilizes key metrics, fundamentals of clinical systems management, evidence-based practice, and Company policies and procedures to support Health and Wellness performance. The HWS will work directly with Wellness Directors and Executive Directors to ensure delivery of care services occurs at the highest standard.

ESSENTIAL DUTIES AND RESPONSIBILITIES

In addition to all components of the above position summary the following "Essential Duties and Responsibilities" are essential to successful performance in this position.

Essential Functions

  • Develop and implement strategic initiatives that align operational policies with business objectives, driving successful outcomes.
  • Responsible for support, training, and execution of all Health and Wellness programs in assigned communities.
  • Audits communities to ensure that Wellness departments are following State regulations and Savant Senior Living policy and procedures.
  • Consults on clinical questions related to resident care and community acuity.
  • Maintains community care revenue and expenses related to delivery of care services.
  • Assists in recruiting, on-boarding and training of new Wellness leadership including Wellness Directors and other Wellness team members as identified.
  • Maintains effective, regular and professional communications with VP of Health and Wellness, Wellness Directors, Executive Directors and others as needed to support the Health and Wellness department.
  • Supervises Wellness Department, managing schedules, delegating assignments, and properly allocating time based on need and priority of the organization during vacancies of a Wellness Director.
  • Participates in the development of Health and Wellness standards, systems, policies and procedures with VP of Health and Wellness and other leaders, articulating the resources (time, money and staff) required to achieve desired outcomes. Identifies barriers to success and communicates those barriers early and often, with recommended solutions.
  • Assist Marketing with referrals of assessments to be completed for communities
  • Travels to Savant communities based on need and priority. Travel up to 75% of the time; travel assignments may be for extended periods of time and may frequently change; flexibility and availability are required. Adheres to company travel policies and guidelines.
  • Completes detailed recaps as needed throughout and at the end of assignments, including making recommendations for improvement in the Wellness Department
  • Maintains a professional approach consistent with Savant conduct expectations and promotes positive
  • representation of the company while traveling and working in communities.
  • Other duties as assigned.
  • Supports communities with assessments


SUPERVISORY RESPONSIBILITIES

This role supervises Wellness Directors of assigned communities.

EDUCATION/EXPERIENCE/CREDENTIALS

  • Multi-site experience preferred.
  • 2+ years working in RCFE or Assisted Living in clinical manager role. May consider years of nursing experience worked in SNF or acute care, dementia, and behavioral management.
  • Valid state LVN/RN license.
  • Proficient with community software systems including Microsoft Office, August Health, Quick MAR, UKG. Other EHR, eMAR and incident management systems experience considered.
  • Multi-site business operations experience.
  • Documented success in exceeding business financial goals.
  • Excellent reputation in management and leadership.
  • Will be required to travel extensively on behalf of Company.
  • Position requires driving responsibilities (may use Company provided vehicle and/or personal vehicle).
  • Must possess valid driver's license.
  • Must satisfactorily meet and be in compliance with Savant motor vehicle policy standards.
  • Able to influence the actions and opinions of others in a desired direction. Exhibits judgment in leading others to meet worthwhile objectives.

PayScale $115,000-$125,000 /year DOE
