Description
Resident Care Manager (RN)What You'll DoAs the Resident Care Manager, you will:
- Develop, implement, evaluate, and update individualized, person-centered care plans.
- Complete comprehensive assessments for new admissions and significant changes in condition.
- Partner with physicians, therapists, social services, and interdisciplinary team members to coordinate exceptional resident care.
- Participate in daily clinical discussions and care conferences to ensure optimal outcomes.
- Utilize the electronic medical record (EMR-POINT CLICK CARE) to accurately document assessments, interventions, physician orders, and resident progress.
- Monitor residents' health status and proactively identify changes in condition.
- Communicate resident condition changes using the SBAR communication process.
- Review laboratory and diagnostic results and coordinate appropriate follow-up.
- Implement physician orders and ensure timely completion of treatment plans.
- Educate residents and families about diagnoses, treatments, medications, and care plans.
- Provide emotional support and guidance to residents and their loved ones.
- Ensure medical records are complete, accurate, and survey-ready.
- Foster a culture of teamwork, compassion, accountability, and outstanding customer service.
- Maintain reliable attendance and contribute positively to the clinical team.
Additional Responsibilities- Coordinate discharge planning and smooth transitions to the next level of care.
- Collaborate with hospitals, physicians, insurance providers, case managers, and community partners to facilitate admissions and discharges.
- Communicate anticipated discharge needs with the interdisciplinary team.
- Promote regulatory compliance by following company policies, professional standards, and all applicable federal and state regulations.
- Perform additional duties as assigned to support resident care and facility operations.

EducationDegree in Nursing from an accredited nursing school. RN required/preferred.
Licenses/Certification Current RN license.
Valid CPR certificate.
ExperienceSix months experience in a long-term care environment preferred.
Minimum, one (1) year of experience as a supervisor in a hospital, nursing care facility, or other related health care facility preferred.
Salary Description
$45-55 per hour dependent on experience
