Responsibilities- Evaluate patients and provide personalized physical therapy treatments that help patients improve mobility, strength, balance, and functional independence
- Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals
- Deliver hands-on therapy interventions designed to support recovery, increased endurance, and enhance overall quality of life in older adults
- Help patients restore functional mobility and safely perform daily activities with greater confidence and independence
- Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful patient outcomes and continuity of care
- Track patient progress, maintain accurate clinical documentation, and adapt treatment plans based on patient needs and rehabilitation goals

Qualifications:- Licensed Physical Therapist (PT) or license eligible in the state of practice
- Graduate of an accredited Physical Therapist Program
- Interest or experience in skilled nursing, long-term care or geriatric rehabilitation
- Passion for patient-centered care and improving quality of life for older adults
