Qualifications

• Current Certificate of Competence with the College of Nurses of Ontario as RN
• Availability to work full-time days with alternating weekends and on-call responsibilities
• Updated immunization records and clear criminal background check required
• Willingness to complete necessary SE Health certifications
• Current CPR certification required
• Ability to perform physically demanding tasks, including various movements and grips
• Required to travel to specific areas including Mount Forest and surrounding regions.

Responsibilities

• Complete comprehensive client assessments and develop personalized care plans
• Act as primary provider for a designated client caseload, leading care coordination
• Implement and evaluate care strategies based on best practices
• Support clients in transitioning to community support services
• Collaborate with other care team members to ensure comprehensive client care
