Job DescriptionA Licensed Registered Nurse (RN) who, under the supervision of the Clinical Manager, coordinates the timely and accurate flow of clinical information during the referral process. The Home Health Intake Liaison serves as a key point of contact between referral sources, patients, and the agency. This role performs utilization review with a high level of expertise by using criteria that demonstrates medical necessity, physician documentation and knowledge of ICD 10 to achieve reimbursement for services and ensuring appropriate utilization of hospital resources. This role supports the intake process, coordinates referrals, ensures order accuracy, and builds strong relationships with hospitals, physicians, and community partners to drive census growth and smooth patient transitions into home health services.

Required Experience- California RN License
- Bachelor's Degree of Science in Nursing
- (BLS) Basic Life Support issued by the American Heart Association
- Driver License, Valid and in State
- Automobile Insurance
- 3 years' experience as an RN
Preferred
- Home Health or Hospice experience
- Knowledge of Medicare, Medicaid, and commercial insurance guidelines strongly preferred
