Registered Nurse Case Manager (RNCM) - Hospice
Schedule: Full-Time | Monday-Friday, 8:00 AM-5:00 PM | Occasional evenings/weekends
Salary: $85,000-$100,000 per year

Key Responsibilities:

As a Registered Nurse Case Manager, you will play a vital role in our interdisciplinary care team, ensuring that each patient receives personalized, high-quality care.
  • Conduct patient admissions and ongoing nursing assessments
  • Develop and implement individualized plans of care based on each patient's needs and goals
  • Collaborate closely with hospice physicians and the interdisciplinary team (IDT)
  • Supervise and coordinate care with home health aides, social workers, and chaplains
  • Educate patients and caregivers about disease progression, symptom management, and available resources
  • Ensure timely and accurate documentation in compliance with hospice standards
  • Promote a culture of excellence, empathy, and accountability in every interaction

Qualifications:
  • Current Registered Nurse (RN) License in Pennsylvania (required)
  • Clinical nursing experience (hospice or home health preferred)
  • Strong communication, organizational, and interpersonal skills
  • Valid driver's license, reliable vehicle, and current auto insurance
