Location: Home Health Offices: 12801 Westlinks Drive, Fort Myers FL 33913

Department: Nursing/Home Health

Work Type: Full Time, 80 hours bi-weekly (Salary/Exempt)

Shift: Days, 8:00:00 AM to 4:00:00 PM

Minimum to Midpoint Pay Rate: Range: $33.37 - $45.05 / hour +

Eligible for up to $10,000 in Hiring Incentives!

Central Team: Central Fort Myers/Lehigh

This is a driving role, visiting patients in the home. Clinical/hands-on patient care and case managing, strong time management and documentation skills needed for this role.

Key Responsibilities:

  • Conduct comprehensive assessments, including OASIS, to establish tailored care plans.
  • Collaborate with a multidisciplinary team to ensure seamless and efficient patient care.
  • Engage in meaningful education and counseling for patients, caregivers, and families.
  • Implement specialized nursing treatments and preventive measure.
  • Document progress and coordinate services, ensuring all team members are informed of patient updates.
  • Supervise LPNs and home health aides, fostering a supportive environment for all staff.
  • Participate in professional development and quality improvement initiatives.

Summary

Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Assists with directing and coordinating nursing care for patients based on established clinical nursing practice. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Supports the organization vision and mission. Uses knowledge of patient's age and cultural diversity into the provision of patient care. Contributes to the provision of quality nursing care through excellence in customer service and performance improvement techniques that demonstrate positive outcomes in patient care.

Teamwork, flexibility and professionalism, while important in all functions of the System, are central to the success of the Home Health Team, and are expected in this role. Duties will include covering weekends and team member PTO, requested office tasks, and traveling outside normal work area regions to meet the needs of our patient caseload and the needs of the organization.

Requirements

Education: Graduate of an accredited school of professional nursing.

Experience: Minimum one-year acute care experience required. Minimum 2 years of cardiac experience preferred.

Certification: BLS (American Heart Association/Basic Life Support). (active within 90 days of start/hire date) Required.

License: Currently licensed RN in the State of Florida.

Other: Communication and interpersonal skills, basic computer skills, and customer service skills required.
