Compensation: $85,000 - $130,000 annually plus

What You'll Do- Manage a home health caseload throughout St. Paul, with on-call and weekend visits within approximately 35 miles of your home.
- Complete OASIS assessments and individualized plans of care.
- Deliver skilled nursing care and patient education.
- Collaborate with physicians and interdisciplinary team members.
- Help patients safely recover and remain independent at home.
- Maintain accurate, timely clinical documentation.
What We're Looking For- Active Minnesota Registered Nurse (RN) license.
- 3+ years of RN experience (home health experience preferred).
- Strong clinical judgment and critical thinking skills.
- Excellent communication and organizational abilities.
- Reliable transportation for local travel.
