REGISTERED NURSE (RN) - Home Health SummitWest Care | Mesa County, COFull-Time or Part-Time | Monday-Friday Day Schedule + On-Call Rotation$35-$43/hour (DOE)
+ Mileage ReimbursementWhat We're Looking For- Active Registered Nurse (RN) License in the State of Colorado (or active NLC Compact License)
- Valid driver's license, reliable transportation, and auto insurance
- Strong assessment and critical thinking skills
- Ability to work independently and manage a structured caseload
- Clear, professional communication with patients, families, and team members
- Comfort with documentation and EMR systems (HCHB preferred)
- Home health experience a bonus, but not required
What You'll Do- Perform comprehensive patient assessments
- Develop and update individualized plans of care
- Provide skilled nursing care and appropriate interventions
- Monitor patient progress and communicate changes to the care team
- Educate patients and families regarding medications, treatments, and care plans
- Coordinate care with LPNs, therapists, caregivers, and providers
- Complete timely and accurate EMR documentation
- Participate in on-call rotation
