REGISTERED NURSE (RN) - Home Health

SummitWest Care | Mesa County, CO
Full-Time or Part-Time | Monday-Friday Day Schedule + On-Call Rotation
$35-$43/hour (DOE) + Mileage Reimbursement

What We're Looking For
  • Active Registered Nurse (RN) License in the State of Colorado (or active NLC Compact License)
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong assessment and critical thinking skills
  • Ability to work independently and manage a structured caseload
  • Clear, professional communication with patients, families, and team members
  • Comfort with documentation and EMR systems (HCHB preferred)
  • Home health experience a bonus, but not required

What You'll Do
  • Perform comprehensive patient assessments
  • Develop and update individualized plans of care
  • Provide skilled nursing care and appropriate interventions
  • Monitor patient progress and communicate changes to the care team
  • Educate patients and families regarding medications, treatments, and care plans
  • Coordinate care with LPNs, therapists, caregivers, and providers
  • Complete timely and accurate EMR documentation
  • Participate in on-call rotation
