Registered Nurse (RN) – Case Manager

Schedule:Full Time Days; On Call requirements

How you’ll contribute

A Registered Nurse (RN) who excels in this role:

• Consults regularly with nursing staff and the multidisciplinary team to evaluate patient status and appropriateness of care, including admission, length of stay, transfers and discharge.
• Monitors patient and family satisfaction; responds to questions and complaints
• Leads and coordinates discharge planning, including patient transfers to other facilities and referrals to community resources; provides discharge education.
• Performs chart reviews
• Accurately documents case management activities, plans and outcomes.
• Supports department operations by working alternate shifts/units as needed; may be assigned on-call.

What we’re looking for

Applicants should have an active Virginia RN license (single-state) or compact license. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins. 

Additional requirements include:

Basic Life Support certification is required within 30 days of hire

Previous RN/Case Management experience is preferred

3 years of RN clinical experience preferred

Hourly Range:  $33.19 - $44.81
