Pediatric Registered Nurse (RN) Care Manager to join our growing team. This full-time position is ideal for a pediatric nurse who values autonomy, clinical excellence, and the opportunity to make a meaningful impact in the lives of children and their families.
This role is approximately
75% field-based and will oversee a pediatric caseload throughout
Fairfield County, Connecticut.
Key Responsibilities- Ensure the highest quality of care for pediatric clients in the home setting
- Develop, implement, and oversee individualized care plans
- Obtain and manage physician orders in accordance with state regulations
- Communicate changes in clients' medical or mental conditions to physicians and the care team
- Conduct nursing assessments and supervisory visits
- Support, mentor, and supervise field staff
- Promote client independence, safety, and well-being within the home environment
- Maintain compliance with agency policies and state regulations
Qualifications- Current Connecticut Registered Nurse (RN) license in good standing
- BSN preferred
- Valid BLS Certification
- Minimum of two (2) years of recent pediatric home care experience
- Recent hands-on pediatric experience with advanced clinical skills, including:
- Ventilator management
- Tracheostomy care
- G-tube/PEG tube management
- IV therapy and infusions
- Wound care
- Strong clinical assessment, communication, and organizational skills
- Ability to work independently and effectively manage field-based responsibilities
Compensation & BenefitsCompetitive Salary$110,000 - $130,000 annually, based on experience
