Full Job Description
Overview
The hours for this position are Monday through Friday 10:30 am to 7pmTwo years experience in the long term care setting preferred. ResponsibilitiesThe RN Registered Nurse Admissions and Discharges is primarily responsible for the following job duties:
  • Assists with ensuring rooms are ready for new admissions

  • Greet new patients warmly and orient them to their new environment

  • Complete admission assessments and documentation, including initiation of the baseline care plans

  • Communicate patient status with interdisciplinary team

  • Complete medication reconciliation and order entry

  • Obtain consents if needed

  • Assist with patient and caregiver education

  • Ensure discharge orders are accurate

  • Complete discharge documentation

  • Complete comprehensive discharge education on post-discharge instructions, including wound care, or new medications.

  • Coordinate post discharge transition of care appointments

  • Complete post discharge follow up calls and documentation

  • Help identify patients at high risk for rehospitalization and implement early interventions

  • Work collaboratively with interdisciplinary team members to ensure smooth transition

  • Complete admission paperwork with the resident/resident representative (MA 401)

  • Assist with other duties as assigned
