As a Registered Nurse Behavioral Health, you’ll contribute to our success in the following ways:
- Assess patients' physical, emotional, social, and environmental needs.
- Manage a caseload and develop, coordinate, and update individualized care plans.
- Complete initial assessments and provide ongoing evaluations during patient visits.
- Monitor patient progress and communicate changes with physicians, supervisors, and the care team.
- Update care plans based on patients' changing needs.
- Educate patients, families, and caregivers about the plan of care and behavioral health support.
- Teach behavioral health care and symptom management to staff in partner care settings.
- Supervise and support Behavioral Health Aides and LPNs while ensuring quality care.
- Report safety concerns or other issues to supervisors as needed.
- Provide compassionate end-of-life support and complete bereavement assessments.
What is Required?
- Current State unrestricted license as a Registered Nurse.
- Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
- 1+ year of experience as a nurse in a clinical care setting.
- Home Health experience is preferred, but not required.
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.

Benefit eligibility can vary by position and shift status. "Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations." The base salary for this position is $86,000 to $92,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.Full compensation details, including total rewards and applicable benefits, will be discussed during the interview process
