Compensation: $75,000 - $100,000 per year plus $4,000 Sign-On Bonus

What You'll Do• Provide
skilled nursing visits in patient homes and care settings.
• Deliver treatments including
wound care, medication management, and assessments.
• Coordinate care with physicians, therapists, and interdisciplinary teams.
• Educate patients and families on
care plans, medications, and recovery goals.
• Maintain
accurate, timely documentation in the EMR.
What We're Looking For• Active Minnesota Registered Nurse (RN) license.
• Experience in
home health, med-surg, TCU, rehab, or clinical nursing.
• Strong clinical judgment and ability to work independently.
• Excellent communication and patient education skills.
• Valid driver's license and reliable transportation.
