- Be the Expert: Assess patients with complex health needs, develop personalized care plans, and ensure the best outcomes.
- Make a Connection: Educate and guide patients and families through their healthcare journey - helping them understand their conditions and the path to better health.
- Collaborate for Success: Work closely with a team of VNA clinicians and external providers to ensure seamless care and optimal results for every patient.
- Document with Purpose: Record and track changes in patient conditions, updating care plans to reflect their progress and needs.
QualificationsWhat We're Looking for in You:We're searching for a nurse who's not only skilled but also compassionate and motivated to help patients achieve their health goals. If you're ready to make a meaningful impact on people's lives, we want to hear from you.
- Registered Nurse (RN) with a Bachelor's Degree in Nursing (preferred).
- 1+ year of clinical experience as a registered nurse (bonus points if you have experience in homecare!).
- Strong verbal and written communication skills - you'll be guiding and educating patients and families.
- Proficient with technology - navigating EMR systems and using digital tools to enhance patient care.
- A reliable car and valid driver's license - you'll be traveling to patient homes, so your own transportation is key.
Physical Demands/Working Conditions:- Flexibility to work both indoors and outdoors while using clinical equipment.
- Comfortable using a phone, computer, and other digital tools to stay connected with your team and patients.
- You'll be making a direct impact through client and customer contact, so your presence is invaluable.
