- Territory covers the Plainsboro and West Windsor areas
Position Overview
- RN Case Manager role within a home health setting
- High-volume position - expect to see 5-6 patients per day
- High volume of documentation required
Required Experience
- Minimum 2+ years of home health experience
- Case management experience required to be considered
- OASIS experience required to be considered
Preferred Experience
- Wound care experience preferred
