Responsibilities

Assess and monitor physical, emotional, and psychological needs of patients.
Create individualized hospice care plans reflecting patients' goals and wishes.
Administer nursing care including medications, treatments, and interventions.
Provide effective pain and symptom management to patients.
Educate and support families and caregivers of patients.
Collaborate with interdisciplinary team members to optimize patient care.
Ensure accurate and timely documentation of patient interactions.
Participate in on-call rotation as needed.

Schedule: Mon-Fri, 8:00 AM- 5:00 PM

As a RN Case Manager You will:

Assess/monitor physical, emotional, and psychological needs of patients

Collaborate with interdisciplinary team

Qualifications

- Graduate of an accredited nursing school with current licensure to practice in state of operation
- One year nursing experience required, 3+ years preferred
- Experience in hospice or a similar setting preferred
- Knowledge of the hospice philosophy of care
- Commitment to clinical and documentation excellence
- Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
