Job Description
Facilitates the patient's hospitalization from admission through discharge. Coordinates with physicians, other nurses and social workers and other disciplines within the care team, including outside agencies, to expedite the appropriateness, effectiveness and timeliness of care.

Job Responsibility

  • Provides health education; stratify and Manage risk
  • Coordinate care with internal and external providers
  • Actively participate with a multidisciplinary team
  • Conduct assessments and Develop treatment plans
  • Applies clinical expertise and medical appropriateness criteria to resource utilization and discharge planning, and manages the resources necessary for cost effective, quality patient care. Coordinate care with internal and external providers with a wide range of professionals including psychiatrists, nurse practitioners, community support specialists, nurses, on-site pharmacist, social workers, licensed professional counselors
  • Partner with the Access team to triage new and existing clients, respond to emergent and urgent medical and psychiatric needs, and provide direct care

Job Qualification

  • Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
  • Current License to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.
