Responsibilities
Assess patients' nutritional status clearly and thoroughly.
Formulate personalized nutrition care plans for each patient.
Coordinate the implementation of nutritional care plans across the medical team.
Evaluate the effectiveness of nutrition care and adjust plans as necessary.
Provide nutrition education to patients and their families.
Approve nutritional supplements that fit the needs of patients.
Collaborate in developing a hospital formulary for the patient population.

Overview
Provides appropriate patient nutritional care. Assesses the nutritional status of patients, formulates a care plan, coordinates its implementation, and evaluates its impact and effectiveness. Provides nutrition education to patients/families. Approves supplements as appropriate and assists in the development of a hospital formulary appropriate to the patient population.
