Role Scope: The Quality Improvement Analyst supports Habitat Health's quality improvement infrastructure across PACE centers. This role maintains core quality data systems, supports participant safety and regulatory reporting workflows, facilitates quality committee operations, and partners with center and central leaders to identify trends, close gaps, and drive measurable improvement.
Core Responsibilities & Expectations for the RoleQuality Reporting - Maintain quality dashboards and CMS/State reporting tools; flag metric gaps, trends, and follow-up needs.
- Prepare monthly metric summaries and follow-up lists that help Center teams stay accountable to quality goals.
- Support quarterly data submissions, regulatory review preparation, and case-level documentation in partnership with Compliance.
Participant Safety & Incident Management - Track participant safety events, support investigations and root cause analyses, and prepare readouts for Center and central leadership.
- Audit whether safety events are captured, escalated, and documented appropriately. Coordinate with Compliance when gaps are identified.
Acute Care Utilization Monitoring - Review ED and inpatient admissions to maintain utilization and quality metrics across Centers.
- Identify utilization trends and surface actionable opportunities to Center teams and leadership.
Quality Improvement Committee (QIC) Support - Support QIC operations, including scheduling, agenda development, data presentation, strategic recommendations, and operational improvements.
- Support annual QIC review and governing body approval for each Center's Quality Improvement Program.
Audits - Execute internal quality audits on a rotating schedule and maintain documentation for regulatory readiness.
- Escalate audit findings that require compliance involvement, corrective action planning, or external audit support.
Required Qualifications: - 2+ years of experience in quality improvement, healthcare operations, compliance, or a related field
- Highly organized, with strong follow-through across recurring workflows, deadlines, and cross-functional partners
- Comfort working with data - maintaining trackers, identifying trends, and translating findings into clear recommendations
- Strong written and verbal communication skills; able to present findings and facilitate meetings with confidence
- Proficiency with Excel or similar tools for tracking, analysis, and reporting

Nice to have: - Clinical background helpful but not required
- Experience in PACE, Medicare/Medicaid managed care, or programs serving older adults with complex needs preferred
- Familiarity with PACE regulations, DHCS requirements, HPMS reporting, or quality/compliance committee operations preferred

Compensation: The anticipated base salary range for this position is $81,000 - $107,000. This position is also eligible for a target annual bonus of 10% of base salary and may be eligible for additional incentive compensation, as applicable.
The final compensation offered will be determined based on factors including, but not limited to, the candidate's qualifications, skills, experience, education, certifications, geographical location, and business needs.
