At Mattapan Health & Rehabilitation Center, rehabilitation is about more than improving strength or completing exercises — it’s about helping residents regain the mobility, confidence, and independence they need to participate in the activities and routines that matter most.  We are seeking a compassionate and clinically driven Physical Therapist (PT) to join our rehabilitation team. In this role, you'll provide skilled physical therapy services, develop individualized treatment plans, and work closely with residents to help them achieve meaningful functional goals.  Our 85-bed skilled nursing and rehabilitation community provides both Short-Term Rehabilitation and Long-Term Care, giving you the opportunity to work with residents recovering from illness, injury, surgery, hospitalization, or functional decline, as well as those who benefit from ongoing maintenance therapy.

 

As a valued member of our interdisciplinary team, you'll collaborate with Physical Therapist Assistants, nursing staff, physicians, and other healthcare professionals to support resident progress, promote safe mobility, and improve quality of life through personalized care.

Responsibilities What You'll Do

As a Physical Therapist, you will evaluate residents, develop individualized plans of care, and provide skilled physical therapy interventions designed to improve mobility, strength, balance, safety, and functional independence.

Physical Therapist Responsibilities:

• Perform comprehensive physical therapy evaluations and assessments based on resident needs
• Develop individualized treatment plans and measurable therapy goals in accordance with physician orders
• Provide skilled physical therapy treatments within the scope of practice and established plan of care
• Treat residents with a variety of orthopedic, neurological, and functional mobility needs
• Help residents improve strength, balance, coordination, endurance, mobility, and functional abilities
• Implement therapeutic exercises, functional activities, gait training, balance interventions, and other evidence-based treatment approaches
• Monitor resident response to treatment and modify interventions as clinically appropriate
• Evaluate resident progress and make recommendations regarding continued therapy, discharge, or maintenance needs
• Provide education to residents, families, and caregivers regarding exercises, mobility strategies, safety, and home or discharge recommendations
• Collaborate with Physical Therapist Assistants regarding treatment plans, resident progress, and clinical recommendations
• Participate in interdisciplinary care planning and discharge planning activities
• Maintain accurate, timely clinical documentation, treatment records, and required departmental reports
• Communicate resident progress, changes in condition, and treatment recommendations to appropriate members of the care team
• Assist with maintaining a safe, clean, and organized therapy environment
• Participate in continuing education, in-service training, and departmental meetings
• Provide clinical direction and support to Physical Therapist Assistants and other therapy staff as appropriate
• Maintain resident confidentiality, privacy, and dignity in all interactions
• Follow all safety, infection control, and regulatory standards
• Perform other duties as assigned

From Up to Qualifications What We're Looking For

Physical Therapist Qualifications:

• Graduation from an accredited Physical Therapy program required
• Current Massachusetts license to practice as a Physical Therapist required
• Current CPR/BLS certification required
• Long-term care, skilled nursing, or rehabilitation experience preferred
• Experience working with geriatric and subacute rehabilitation populations preferred
• Strong clinical reasoning, evaluation, and treatment planning skills
• Ability to develop and implement individualized plans of care
• Strong interpersonal and communication skills
• Ability to work collaboratively with residents, families, and interdisciplinary healthcare teams
• Ability to provide clinical direction and support to Physical Therapist Assistants as appropriate
• Compassionate, dependable, and resident-focused approach to care
• Ability to meet applicable state and federal regulatory requirements
