The Psychiatric Nurse Practitioner plays a vital role in advancing Harbor Care's mission and vision by providing patient-centered, evidence-based psychiatric evaluation and medication management for adult patients served by Harbor Care Health & Wellness Center. This position is responsible for delivering comprehensive psychiatric care to adult patients, including diagnosis, medication management, treatment planning, and collaboration with the interdisciplinary care team.

The Psychiatric Nurse Practitioner provides on-site services while seeing patients, with 32 hours of bookable patient-facing hours per week and 8 hours of administrative time that can be taken off-site. While telehealth may be used to provide access for certain patients, the primary mode of care will be face-to-face encounters. Candidate will ensure that services align with Harbor Care's philosophy of care and values of compassion, respect, collaboration, and transparency to create a positive impact on those we serve and the broader community.

What You'll Do

• Provide psychiatric evaluations, diagnostic assessments, and medication-management services to adult patients ages 18 and older.
• Diagnose and treat psychiatric conditions within the psychiatric nurse practitioner scope of practice.
• Collaborate with primary care providers, therapists, case managers, nursing staff, and community partners to coordinate integrated patient care.
• Maintain timely, accurate, and complete clinical documentation in the electronic health record.
• Participate in team meetings, case consultation, quality-improvement activities, and other organizational initiatives.
• Duties and responsibilities may be adjusted to ensure alignment with funding requirements and organizational objectives.

Requirements

What We're Looking For

Education:

• Master's degree or higher in nursing from an accredited Psychiatric-Mental Health Nurse Practitioner program.
• National certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) through the American Nurses Credentialing Center (ANCC)
• New Hampshire prescriptive authority and DEA registration

Experience:

• Experience providing psychiatric medication management for adults with substance use disorders and co-occurring psychiatric conditions preferred.
• Experience serving diverse, low-income, uninsured/underinsured, homeless, or otherwise underserved populations as preferred.
• Experience in outpatient behavioral health, community mental health, integrated care, or an FQHC preferred.

Certifications/Licensure:

• Current, unrestricted New Hampshire APRN license or eligibility for licensure; current RN licensure as required.
• Current BLS certification and additional behavioral health or substance use disorder training/certification preferred.
