Full Job Description
Job Summary:

Supports the implementation and coordination of patient safety activities at the hospital or regional level. Working under the direction of patient safety leaders, this role assists with initiatives to reduce preventable harm and improve reliability of care processes. Serves as a resource for staff, helping to coordinate root cause analyses, reinforce high reliability skills, and support action plans. Assists system leaders with local safety activities and supports survey readiness and improvement in patient safety practices.

Job Requirements:

Education and Work Experience:
  • Bachelor's degree in Health Care Administration, Nursing, Public Health, or related field or equivalent combination of education/related experience: Required
  • One year experience in quality, patient safety, risk management, or performance improvement.: Required
  • Two years experience in patient safety, quality improvement, or related roles: Required


Licenses/Certifications:
  • Certified Professional in Patient Safety (CPPS) certification or Lean certification: Required within two years of hire
  • Registered Nurse (RN) license in the state of practice or related credential: Preferred
  • CPPS or Lean or Six Sigma Green Belt (LGB): Required


Essential Functions:
  • Supports the local patient safety program in alignment with system goals. Assists with deployment of high reliability skills and patient safety curriculum for associates and staff. Helps monitor and report on progress toward safety goals.
  • Leads and facilitates root cause analyses, apparent cause analyses, and FMEAs for significant safety events. Develops, implements, and tracks corrective action plans to ensure closure and sustainability. Partners with clinical and operational leaders to proactively identify and mitigate safety risks.
  • Oversees culture of safety surveys, analyze results, and lead action planning. Supports Care for the Caregiver, Communication and Resolution Program, and other workforce and patient engagement safety initiatives. Promotes consistent event reporting, classification, and learning.
  • Provides education on patient safety principles, high reliability behaviors, and error prevention strategies. Supports leaders and frontline teams in reliability practices and safety improvement methods. Recognizes and spreads safety best practices across departments.
  • Helps compile safety event data and assist with identifying trends. Supports sharing of learnings from the Patient Safety Organization (PSO). Assists with preparing reports for leadership and quality committees.
  • Performs other job-related duties as assigned.
