What you will do:- The Primary Nurse Clinician provides expert clinical leadership and delivers high-quality, client-centered care to individuals with complex needs in the community.
- This role supports safe, timely care delivery while strengthening team capability and care continuity.
- Deliver advanced nursing care for complex patients, including urgent, time-sensitive visits and care plan adjustments.
- Conduct comprehensive, evidence-based assessments and develop individualized care plans in collaboration with clients, families, and interdisciplinary teams.
- Mentor, support, and supervise team members to enhance clinical skills, confidence, and professional growth.
- Lead accurate, timely clinical documentation and ensure compliance with regulatory and organizational standards.
- Champion innovative, client-centered care models that support hospital avoidance, safe transitions, and continuous quality improvement.
What you will need to be successful:- Current registration with the applicable provincial regulatory body.
- A minimum of 3-5 years of supervisory experience or equivalent clinical leadership experience in home and community healthcare.
- Advanced Clinical Expertise: Demonstrated ability to deliver acute and advanced care in the home or community setting.
- Strong Leadership Skills: Proven track record in team development, supervision, and fostering collaboration.
- Exceptional Organizational Skills: Capacity to manage time-sensitive visits, prioritize patient needs, and maintain comprehensive records.
- Commitment to Quality Care: Passion for delivering high-quality, safe, and patient-centered services.
- Strong proficiency with Microsoft Office Suite and experience with scheduling/documentation systems.
- A valid Alberta driver's license and access to a reliable, insured vehicle.
- Clinical Expertise: Strong assessment, care planning, and medication reconciliation skills.
- Problem Solving: Analytical and critical thinking to identify service gaps and implement improvements.
- Communication: Excellent verbal and written skills for interacting with diverse stakeholders.
