This is a full-time, on-site, patient-facing role. You report to the Associate Medical Director and are responsible for both in-visit care and ongoing patient management.
What You'll Do- You will see 12-15 patients per day, allowing for longer, more in-depth visits
- You will manage patients with complex and chronic conditions
- You will complete accurate, same-day documentation to support patient care and outcomes
- You will manage a daily clinical inbox (labs, messages, follow-ups)
- You will coordinate care with specialists and care team members
- You will apply Medicare risk adjustment principles through accurate condition documentation

Use your skills to make an impact Required Qualifications- MD or DO from an accredited program
- Board Certified or Board Eligible in Family Medicine, Internal Medicine, or Geriatrics
- Active, unrestricted medical license (or ability to obtain) in South Carolina
Additional Experience We Value- 2+ years of clinical experience with senior populations
- Experience with Medicare Advantage or risk adjustment
- Experience supervising Advanced Practice Providers (NPs/PAs)
- Proficiency with electronic health records (EHR) and digital documentation.

Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$255,000 - $298,500 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
