Full Job Description
The newest Primary Care Physician at the Colorado Coalition for the Homeless (CCH) will join a group of experienced, dedicated, and compassionate medical providers to provide high-quality primary care to some of the most vulnerable patients in the Denver metro area. This physician will care for individuals currently or formerly experiencing homelessness in fast-paced, exciting, and varied environments. No two days are the same. This position will serve across various programs including the Stout Street Health Center, the Recuperative Care Center, Street Medicine, home visits, Health Outreach Program (mobile clinic), and satellite clinics. The physician will deliver care using the principles of motivational interviewing, harm reduction, and trauma-Informed care to support patients in achieving better health outcomes.
Duties/Responsibilities
37 Provide direct, high-quality primary medical care to people experiencing homelessness.
37 Exhibit a positive attitude and a commitment to social justice in all aspects of patient care.
37 Utilize motivational interviewing, harm reduction, and trauma-informed care principles when working with patients.
37 Collaborate effectively with other primary care medical providers as well as the multidisciplinary healthcare team (psychiatry, behavioral health, case management, dental, vision) to ensure comprehensive care.
37 Perform other job duties as assigned.
Education, Experience, and Job Competencies
37 Education
37 Medical Degree or Doctor of Osteopathy degree from an accredited school of medicine.
37 Completion of an accredited Residency Program in the physician's specialty.
37 Board Certified or Board Eligible status in the physician's specialty
37 Experience
37 Demonstrated experience caring for patients experiencing homelessness, substance use disorders, poverty, and mental illness.
37 Current licensure or ability to obtain licensure in the State of Colorado as a physician.
37 Current DEA licensure

Department Overview & Requirements
37 This position is primarily located at 2130 Stout Street, Denver, CO 80205 and may require travel to other clinical sites within the CCH network.
37 Current typical schedule is either four 10-hour shifts or five 8-hour days each week. If four 10-hour shifts are preferred, the applicant should expect to work Monday, Wednesday, and Friday with the option to have either Tuesday or Thursday off.
37 For a 10-hour schedule typical hours are 7:00 AM to 5:00 PM (Mondays, Tuesdays, Thursdays, Fridays) and 8:00 AM to 6:30 PM on Wednesdays.
37 For an 8-hour schedule, typical hours are 7:00 AM to 3:00 PM (Mondays, Tuesdays, Thursdays, Fridays) and 10:00 AM to 6:00 PM on Wednesdays
37 Evening coverage to 6:00 PM on Wednesdays is required.
37 Weekend coverage may be required.
37 Call schedule: after the first year with CCH, the physician is expected to take 2 weeks of at-home call per year (phone calls only, no in-person requirement).
37 Spanish-language proficiency is preferred, but not required.
37 Experience with wound care is preferred, but not required.
