Primary Care NP/PA - Make a Real Impact in Home and Community Medicine

Territory: Aurora, CO

As a Primary Care Provider, you'll be a lifeline for homebound seniors, helping them age safely and comfortably in place. You will provide relationship-based primary care, respond to acute needs, and drive measurable improvements in health outcomes-all with full support from Bloom's interdisciplinary team and advanced data systems.

What You'll Do
  • Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
  • Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
  • Address social determinants of health, including housing stability, nutrition, caregiver support, and access barriers.
  • Conduct specialty visits such as wound care, post-hospital transitional care, and condition-specific assessments.
  • Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
  • Utilize Bloom's integrated data ecosystem to track hospitalizations, coordinate timely post-discharge visits (within 72 hours), and drive superior outcomes.
  • Collaborate closely with internal and external care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
