Position Details:Hartford HealthCare is seeking a Full-time Family Nurse Practitioner or Physician Assistant for a multi-provider, primary care practice seeing patients age 12+. The position is based in Plainfield and Putnam, CT.
Role: Open to Family Nurse Practitioners (FNP) and Physician Assistants (PA)
Locations: Plainfield and Putnam, CT
Setting: 100% Outpatient Primary Care - 12+ years old
Schedule: Full-time, 4 days per week (3 days in Plainfield and 1 day in Putnam)
Call schedule: No call required

Nurse Practitioner and Physician Assistant Scope: Under established guidelines and in collaboration and/or supervision with the practice physician(s), Nurse Practitioners and Physician Assistants perform routine physical examinations, and formulate medical, educational, and psychosocial plans to treat patients.
In consultation with the physician(s) and care team members, Nurse Practitioners and Physician Assistants develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
Nurse Practitioners or Physician Assistants provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.
Qualifications: Requirements: - Licensure : Unrestricted State of Connecticut Medical License (or eligible)
- Training : APRN (Family Nurse Practitioner) or PA
Preferred: - Experience : Two years of post-graduate experience as a Nurse Practitioners (APRN) or Physician Assistants (PA) in Primary Care or a related field seeing both peds and adults
