Position Purpose: This is an on-site position. The Population Health Program Manager reports to the Director of Practice Operations and leads the development, implementation, and performance management of population health initiatives aligned with organizational strategy. This role collaborates with clinical, operational, and analytics leaders to drive care transformation, support physician engagement, and implement programs that improve quality, outcomes, and value across the continuum of care. FLSA Exempt position.

Key Responsibilities:
  • Supports strategic planning for evolving population health plan consistent with organizational priorities and monitors key performance indicators to deliver best in class programs. Partners with key organizational stakeholders to establish and integrate programs.
  • Leads and develops content for key committees that oversee performance and projects and programs supporting the population health portfolio, including interfacing with external partners (payers).
  • Coordinates with analytics team to develop reporting tools to evaluate and develop actionable reports.
  • Responsible for being a care transformation change agent through strong partnerships and collaboration with providers.
  • Aligns critical physician partnerships necessary in such programs by creating a shared vision and fostering a relationship characterized by open communication and trust.
  • Partners closely with operations, analytics, and clinical leadership, including all areas of population health operations, to assure all are informed, working as one unified organization, and aware of deadlines, requirements, and their roles within the population health program infrastructure.
  • Supervises the nurse navigators within outpatient clinics.
  • Performs other duties as assigned to support overall effectiveness of department and organization.

Qualifications:
  • Education: Bachelor's degree in Population Health, Clinical Quality/Clinical Process Improvement, or Nursing
  • Licensure: Current, valid AZ RN License
  • Experience: 3 years of population health, quality/process improvement, nursing leadership, or project management
  • Skills/Knowledge: Excellent communication and presentation skills; effective in working with all levels of clinicians and staff • Demonstrated project management and organizational skills • Robust analytics and problem-solving skills that allow for thoughtful assessment of decisions and associated implications

Work Requirements: Ability to walk up to 4 hours daily and sit up to 5 hours daily. Have visual acuity. Ability to do minor lifting to 25 lbs.
