Full Job Description
Office Location: Baden Business Office
Work Hours: Primarily M-F business hours

The Population Health Pharmacist is responsible for optimizing medication therapy outcomes across targeted patient populations through evidence-based clinical interventions, medication management, and collaborative care strategies. This role supports organizational population health initiatives by improving quality metrics, reducing healthcare costs, enhancing medication adherence, and promoting safe and effective medication use.

REQUIRED
  • Doctorate of Pharmacy
  • Licensed or eligible for licensure in the state of Pennsylvania
  • Must have the ability to expertly communicate verbally and in writing including the use of electronic mail.
  • Must possess excellent human relations skills
  • Must have the ability to interact in a professional manner
  • Must have the ability to delegate and motivate others within the department to act on performance improvement opportunities
  • Must be adaptable to change
  • Must know and understand the law as it applies to outpatient practices


PREFERRED
  • Management experience preferred
  • Experience and knowledge of population health strategy.
  • Ambulatory pharmacy experience
  • Competency with Microsoft Office suite
  • Understanding of value based reimbursement programs
  • Advanced degree in Pharmacy or Business
