Qualifications

• Master of Science degree in Nursing-Adult-Gerontology Primary Care Nurse Practitioner or Family Nurse Practitioner preferred.
• Completion of Primary Care Nurse Practitioner Fellowship Program.
• Graduate of a Nurse Practitioner education program registered by the New York State Education Department.
• Certification as an Adult-Gerontology Primary Nurse Practitioner or Family Nurse Practitioner from an approved accrediting body.
• Current, unrestricted New York State Registered Nurse and Nurse Practitioner licenses required.
• Active Drug Enforcement Agency (DEA) registration or eligibility for registration is necessary.
• BLS or ACLS certification is required.
• Bilingual Spanish preferred.

Responsibilities

• Conduct virtual and in-person visits to address HEDIS gap measures and promote preventive screenings.
• Perform physical exams, evaluate patients, and document findings to mitigate healthcare disparities.
• Diagnose and treat diseases, and educate patients on preventative measures and chronic disease management.
• Prescribe medication in accordance with New York State and federal law.
• Order and interpret laboratory and radiological examinations or tests.
• Provide preventive health services, including education and counseling to prevent readmissions and improve outcomes.
• Collaborate with clinicians to ensure continuity of care, developing metrics to track patient experience outcomes.
• Assist in facilitating the Primary Care Nurse Practitioner Fellowship Program with teaching, curriculum development, and accreditation.
