About the Role:Skilled Nursing Pharmacy in Hayward, CA is looking for a dedicated and knowledgeable Staff Pharmacist to join our team. Responsibilities:- Dispense and verify prescription medications accurately for skilled nursing facility residents
- Perform drug utilization reviews (DUR) and ensure medication therapy appropriateness
- Collaborate with nursing staff and physicians to optimize patient medication regimens
- Monitor for drug interactions, contraindications, and adverse drug reactions
- Ensure compliance with state and federal pharmacy regulations and long-term care guidelines
- Counsel and support facility staff on medication administration and safety protocols
- Maintain accurate patient medication records and documentation
Requirements:- Active California Pharmacist license (RPh) in good standing
- Doctor of Pharmacy (PharmD) degree from an accredited institution
- Experience in long-term care, skilled nursing, or institutional pharmacy preferred
- Strong knowledge of geriatric pharmacotherapy and medication management
- Familiarity with California Board of Pharmacy regulations and OBRA guidelines
- Excellent attention to detail and strong organizational skills
- Effective communication and interpersonal skills for multidisciplinary collaboration
Compensation: $62.00 - $64.00 per hour
