Full Job Description
To Qualify: One must be a board-certified practicing physician with the ability to obtain an unrestricted Missouri medical license.

Job Summary:

We are seeking a highly skilled and compassionate PACE Physician to join our team. The PACE Physician will be responsible for providing primary care services to our elderly patients in a Program of All-Inclusive Care for the Elderly (PACE) setting. The ideal candidate will have a passion for working with the elderly population and a commitment to providing high-quality care.

Key Responsibilities:

- Provide primary care services to elderly patients in a PACE setting

- Conduct comprehensive assessments of patients' medical, social, and functional status

- Develop and implement individualized care plans for each patient

- Coordinate care with interdisciplinary team members, including nurses, social workers, and therapists

- Monitor patients' health status and adjust care plans as needed

- Provide education and counseling to patients and their families

- Participate in quality improvement activities and initiative
