Family Medicine Physician Position Overview

Responsibilities
  • Provide comprehensive outpatient Family Medicine care for patients across the lifespan.
  • Diagnose, treat, and manage acute and chronic medical conditions.
  • Provide preventive care, routine physical examinations, health screenings, and immunizations.
  • Manage common conditions including diabetes, hypertension, hyperlipidemia, respiratory illnesses, and other chronic diseases.
  • Provide age-appropriate health counseling, wellness education, and lifestyle management.
  • Perform in-office procedures as appropriate to training and scope of practice.
  • Coordinate specialty referrals, diagnostic testing, and follow-up care.
  • Collaborate with nurse practitioners, medical assistants, behavioral health providers, and other members of the care team.
  • Maintain accurate, timely, and thorough documentation in the EHR.
  • Participate in quality improvement initiatives and population health programs.
  • Engage in community outreach and health education programs.
  • Support continuity of care and develop long-term relationships with patients and families.
  • Perform other duties as assigned.
