Family Medicine Physician Position Overview
Responsibilities
- Provide comprehensive outpatient Family Medicine care for patients across the lifespan.
- Diagnose, treat, and manage acute and chronic medical conditions.
- Provide preventive care, routine physical examinations, health screenings, and immunizations.
- Manage common conditions including diabetes, hypertension, hyperlipidemia, respiratory illnesses, and other chronic diseases.
- Provide age-appropriate health counseling, wellness education, and lifestyle management.
- Perform in-office procedures as appropriate to training and scope of practice.
- Coordinate specialty referrals, diagnostic testing, and follow-up care.
- Collaborate with nurse practitioners, medical assistants, behavioral health providers, and other members of the care team.
- Maintain accurate, timely, and thorough documentation in the EHR.
- Participate in quality improvement initiatives and population health programs.
- Engage in community outreach and health education programs.
- Support continuity of care and develop long-term relationships with patients and families.
- Perform other duties as assigned.
